PHYSICIANS: To refer patients to our facility please send a written prescription with patient diagnoses.Special test findings such as written X-ray, Cat Scans, MRI reports are also extremely helpful for physical therapy evaluation and treatment of your patients. Faxed patient referrals are also accepted at (305) 867-3927.
