INFORMATION
You may visit our clinic with or without a written physical therapy
referral. Please be aware that most insurance requires a written
prescription and or preapproval for insurance coverage of physical
therapy services.
If your insurance
does not provide physical therapy coverage, we do accept payment
via check, cash, money orders or travelers checks.
We recommend that you verify your insurance benefits for physical
therapy by doing the following:
> Call your insurance
company to verify your therapy coverage. Most insurances allow
you to check in the member section of their web site.
> Investigate yearly dollar coverage limits
or number of approved treatment visits for therapy and if pre-authorization
for treatment is required.
> Record the full name of the insurance contact
person.
Record the pre-authorization number for treatment if available.
> Receive fax or email confirmation of benefits.
> Auto accident injuries should obtain information
from your car insurance company on the limits and extent of their
personal injury protection coverage.
> For auto or work related injuries be sure
to acquire your adjuster's name, phone and your claim number to
expedite authorization for your treatment.
> Please bring your written insurance information
and your physician prescription for treatment with you to your
first therapy appointment.
> You should allow yourself 60-90 minutes for
your first therapy visit. We look forward to helping you.
