We are
a conveniently located on 71st Street one block west of Collins Ave
and we offer free parking.
For
Physicians:
To refer patients to our facility please send a written prescription
with patient diagnoses. Special test findings such as written X-Ray
reports, Cat Scans, MRI reports are also extremely helpful for physical
therapy evaluation and treatment of your patients.
Faxed patient referrals are also accepted at (305) 867-3927
For Patients:
If you are sending email concerning a particular physical therapy
problem, please remember that e-mail is not private. If you would
like to maintain a private patient confidentiality record, please
call and visit our clinic in person.
